As promised I’m coming back with an example for what I believe to be the perfect periodical employee evaluation form. Before jumping to it, you might want to check out my updated article on periodical employee evaluation dos and don’ts.

Evaluation Form

Chapter 1 – Self Evaluation

  1. Do you like your job – Yes/No? If not, please share with us why.
  2. Are you able to do great at your job – Yes/No? If not, please share with us why.
  3. Which of your previously set top 3 professional goals were you able to achieve so far? If you did not achieve all of your previously set goals, please share with us why. And don’t forget to share with us your top 3 professional goals for the following period.

Chapter 2 – Mission & Vision Evaluation

  1. Do you know what we are trying to do here at Example Inc. – Yes/No? If not, why do you think that is?
  2. Do you like what we are trying to do here at Example Inc. – Yes/No? If not, please share with us why.
  3. Do you feel that you are contributing to what we are trying to achieve – Greatly/Fairly/Not at all?
  4. What can we do to help you contribute more?

Chapter 3 – Supervisor/Peer Evaluation

  1. Do you think that John/Jane likes his/her job – Yes/No? If not, please share with us why do you think that is.
  2. Is John/Jane able to do great at his/her job – Yes/No? If not, please share with us why do you think that is.
  3. John/Jane‘s previously set top 3 goals were goal 1, goal 2 and goal 3 . Which of those professional goals was John/Jane able to achieve so far? If he/she didn’t achieve all of the previously set goals, please share with us why do you think that was.

And that’s it. Just 10 questions, but 10 questions that should give you a nice big picture, but also individual feedback from each and every one of your employees – you included.

And it can also be adapted and used on any other type of Enterprise, Agency, Authority, NGO, University et cetera.

Now, seeing that I’ve shared with you my top 7 tips for an excellent firing experience, here’s an essential example of the infamous exit interview. I recommend that you use it regardless of the reason or method of termination.

Exit Interview

  1. Do you like Example Inc. as a company/brand/etc – Yes/No? If not, please share with us, why.
  2. Did you enjoy working for Example Inc. – Yes/No? If not, please tell us why.
  3. Do you think Example Inc. has enjoyed having you as an employee – Yes/No? If not, please share with us, why.

It’s everything that you need, not more, not less…

Sure, I’ve seen exit interviews containing questions like – How did you find out about Example Inc. in the first place?, but that’s a question you should have asked upon hiring, not when your employee is leaving your company.

Or, Would you consider working for Example Inc. again?… If they (still) like Example Inc. and they’ve enjoyed working for you, they would definitely come back if an opportunity were to present itself, if not, it’s safe to assume they won’t…

Disclaimer: Both John and Jane are fictional characters and Example Inc. is evidently a fictional corporation. I used them to better express my ideas and to give my audience a clearer, more expressive picture. If however there is an actual Example Inc. out there and coincidentally both a John and a Jane work for that company, then I take full credit for the hilariousness of the entire matter…
